Comments:
An unannounced monitoring inspection was conducted on 10/18/2023 from 3:40 pm to 5:00 pm. At the time of arrival, there were a total of 25 children, ages 9 to 12 years old, in care with 2 staff. A sampling of 5 children's records were reviewed during the inspection. Staff records are housed offsite and 2 staff records were reviewed digitally on 10/24/2023. Children were observed having an afternoon snack which they bring from home, working on homework assignments, engaged in table top activities, and playing gross motor games in the gymnasium. Hand washing and table sanitizing were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of records and interview the licensee was not able to demonstrate that each child's record contains all of the required documentation to include the date that the child began attending the program.
Evidence:
Five records were reviewed.
The record for child #1 who was in attendance on the date of inspection did not documentation of the child's first date of attendance.
The site supervisor confirmed that the first date of attendance / enrollment date was not present ion child #1's record at the time of the inspection.

Plan of Correction: The site supervisor will determine child #1's first date of attendance and record it in the record for child #1.

Standard #: 8VAC20-780-90--A
Description: Based on a review of records and interview the licensee was not able to demonstrate that a written agreement between the parent and the center , containing the required elements, was signed by the parent of each enrolled child by the child's first date first day of attendance.
Evidence:
Five records were reviewed.
The record for child #1 who was in attendance on the date of inspection did not contain a signed written agreement that includes
1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds;
2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and
3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
The site supervisor confirmed that the signed agreement was not present ion child #1's record at the time of the inspection.

Plan of Correction: The site supervisor will ask the program director to print this portion of the electronic file for child #1's record and will place it in child #1's record.
