Comments:
A religious exempt inspection was conducted on 08/23/2022, between the hours of approximately 10:15 a.m. and 11:50 a.m. There were 65 children present with 14 staff, in 7 groupings, observed during this inspection. Children were observed completing worksheets, coloring, playing in centers, working on puzzles, playing on the playground and eating a snack (cheese and crackers). Positive interactions were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.031-A-4
Description: Based on record review and staff interview, each staff member did not have documentation certifying them by a physician to be free of any disability that would prevent them from caring for children under their supervision.

Evidence: Staff #1 did not have an annual health form certifying them by a physician to be free of any disability that would prevent them from caring for children under their supervision.

Plan of Correction: Staff #1 has an appointment scheduled for 08/31/2022.

Description: Based on record review and staff interview, the center does not have an employee trained to perform a daily health screening and exclusion of sick children.

Evidence: Staff #1 was not aware that they were required to have a staff member trained in daily health observation.

Description: Based on observation, not all areas of the premises, accessible to children, were free of obvious injury hazards.

Evidence:
1. In Room 122 one bleach cleaner spray, one sanitizing spray, and one container of disinfectant wipes were in an unlocked cabinet, accessible to children.
2. In Room 125, one spray cleaner and two disinfectant sprays were on a filing cabinet, accessible to children. One disinfectant spray was on the soap dispenser in the bathroom accessible to children.
3. In Room 131, one bottle of prescription medication and one bottle of over-the-counter medication was in an unlocked cabinet, accessible to children.
4. In Room 107, two spray cleaners, one disinfectant spray and one container of disinfectant wipes were on the brown wooden shelf, accessible to children. Six containers of disinfectant wipes and one disinfectant spray was in an unlocked cabinet, accessible to children. One kettle of water was plugged in by the teacher?s desk.
