Comments:
A monitoring inspection was conducted on 03/21/2024 from approximately 11:15 a.m. to 5:20 p.m. There were 26 children present and 7 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child (5) records and five (5) staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.

Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on child record review, the provider failed to obtain documentation of additional
immunizations once every six months for children under the age of two years.
Evidence:
The record for C4, under the age of 2, enrolled 01/16/2023 did not contain documentation of additional immunizations.

Description: Based on documentation review and interview, the center failed to maintain a current written list of all children allergies, sensitivities, and dietary restrictions that is dated and kept confidential.

Evidence:
Licensing inspector reviewed the child allergy list posted in the center. The allergy list was not dated. S1 and S6 were interviewed and were unable to determine the date of the list, however, did report that multiple children on the list were no longer enrolled at the center. S6 reported that the allergy list should be updated monthly.

Description: Based on record review, the provider failed to keep records for staff as required.
Evidence:
The record for S2, employed 06/05/2023 and identified during the inspection as a program leader did not contained documentation to demonstrate the required experience and 24 hours of training required to be a program leader (8VAC20-780-210).
The records for S3 and S4 did not contain first aid and cardiopulmonary resuscitation certifications.
S5 was observed working with children during the inspection; however, the personnel record for S5 could not be located.

Description: Based on child record review, the provider failed to have a written agreement between the parent and center by the first day of the child?s attendance authorizing medical care, parental acknowledgement requiring pickup of an ill child, and parental acknowledgement to inform the center within 24 hours if the child or any member of the immediate household as developed a reportable communicable disease.

The wooden sidewalk has rotten planking and mulch retainer is broken causing a trip hazard.
The chain link fence is loose from the top rail causing a head entrapment hazard.
Older children playground:

Prescription medication for C6, maintained in the office file cabinet was damp and mold was observed on the prescription box.

Description: Based on observation, the provider failed to ensure hazardous substances such as cleaning material be kept in a locked place preventing access by children.
Evidence:
The laundry room adjacent to two classrooms was open and contained laundry detergent on the floor with the warning label to keep out of reach of children.

dated.
Evidence:
Licensing inspector observed multiple lunch boxes and water bottles in the toddler and 3?4-year-old rooms that were not labeled with the child?s name or dated.
