Comments:
A Subsidy Health and Safety inspection was conducted on 05/29/2019 from 09:20am to 2:22pm to review supplemental health and safety requirements. The children were observed learning through play in centers and on the playground. The infants were observed eating, having tummy time and floor play. A formal observation was completed with the infant and two year old classrooms. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today as well as the facility?s playground and three vehicles.

Five children?s records and five employee records were reviewed during this inspection. Medication is not being administered.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Standard #: 22VAC40-665-540-A
Description: Based on review, the vendor did not ensure that one of five staff records contained documentation that the staff was evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Evidence: The record of staff #4 (start date: 04/06/18) did not have documentation that the staff was evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB).

Plan of Correction: The doctor said he will fax the TB results which were completed in 2018 as soon as the electricity is turned on [due] to a neighborhood electric power outage.
