Provided consultation on:
1) standard 22 VAC 40-185-160B: Acceptable forms of documentation of a negative tuberculosis (TB) screening are 1. A clearance statement signed by a physician, the physician's designee or an official of the local health department; 2. The results of a negative tuberculin skin test (TST); or 3. The results of a chest x-ray negative for tuberculosis disease.
2) 22 VAC 40-185-70A: staff records shall contain the name, address and telephone number of a person to be notified in an emergency

An announced, focused, monitoring inspection was conducted from 1:00 - 4:30 pm offsite at the Massad central office. Four staff records were reviewed. An exit interview was conducted 1/31/2020. Standard #: 22VAC40-185-70-A Description: Based on review of four staff records, the facility did not ensure that each staff record contained all required information. Evidence: the records for staff #1, staff #2 and staff #3 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position. Plan of Correction: Will ensure that education and experience are documented in the records.
