Standard #: 8VAC20-780-160-A Description: Based on a review of staff records, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening (TB) at the time of employment, prior to coming in contact with children and been completed within the last 30 calendar days of the date of employment.

Standard #: 8VAC20-780-280-B Description: Based on observation, the center did not ensure all hazardous substances such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In classroom #2, one container of Clorox wipes labeled " Caution" and "Keep Out Of Reach of Children" was located on top of the cubbies.
In classroom #3, the diaper changing area had Lysol and disinfecting wipes labeled "Caution" and " Keep Out of Reach of Children" accessible to children.
