22VAC40-665 INTRODUCTION

Today we reviewed 63.2(17)-1716-B-7 reports of serious injury or death.
Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. Reports of deaths shall be submitted no later than one business day after the death occurred.

A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on January 28, 2020, from 2:45 p.m. to 4:05 p.m. At the time of arrival the following staff to child ratios were observed:
20 children, four years old, with 2 staff
16 children, three years old, with 2 staff
Classroom observations were made and children were observed playing in free choice learning centers.
Six staff records were reviewed, posted information and parent handbooks were reviewed and the center program director was interviewed regarding the centers policies and procedures. The facility, to include indoor areas used by preschool children and before and after care children, as well as the outdoor play space, were observed.
Staff report that the program does not routinely transport preschool children or children in the before or after school programs in a vehicle.
