A unannounced mandates monitoring inspection was initiated on April 24, 2024 from 12:20pm - 2:37pm. The inspection concluded on April 24,2024. There were 56 children with 12 staff present and additional staff was present. The inspector reviewed compliance in the areas of
administration, physical plant, staffing and supervision, programming, medication, special care
and emergencies and nutrition. A total of six children records and six staff records were reviewed. The children were observed naptime and infant having tummy time. The outside area was inspected.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: REPEAT
Based on record review and staff interview, the licensee did not ensure all staff undergo a finger print background and sworn statement of affirmation every 5 years.
Evidence:
Staff#3 most recent fingerprint background check was 03/20/2019. Background check were more than 5 years old.Plan of Correction: All expired March background checks were submitted.
Standard #: 8VAC20-780-130-A Description: Based on record review, the center did not ensure to obtain documentation that each child has recieved the immunization.
Evidence:
The center was missing immunization documentation for Child #1 ( start date 07/12/2023).Plan of Correction: Parent was contacted and will bring it in today.
Standard #: 8VAC20-780-140-A Description: Based on record review each child shall have a physical examination before the child's attendance or with in 30 day after the first day of attendance.

The center did not have a documentation of physical for Child #1 start date 7/12/2023.Plan of Correction: Parent was contacted and parent will being information today,

Based on observation the licensee did not ensure all children records contain all required information.

Record for child #3 was missing emergency contact addresses..Plan of Correction: Parent will fillout the form at pick up .
Standard #: 8VAC20-780-80-A Description: Based on reord review, the licensee did not ensure for each group of children. the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
Confirmed with staff and person in charge. Reviewing the record in the preschool classroom there was eleven children during nap . There was six children not signed in but was present.Plan of Correction: Retrain staff immediatley about taking attendance.
Standard #: 8VAC20-780-90--A Description: REPEAT
Based on reocrds review , the licensee did not ensure A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance.
Evidence:
Child #1, Child #2, Child #3 and Child #4 did not have two of three agreements documented in their records. The missing agreements were a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.Plan of Correction: Parent will sign at pick up today.
Standard #: 8VAC20-780-450-A Description: Based on record review, the licensee did not ensure cots and mats had a top cover and bottom covers or one piece covering.
Evidence:
In the Pre-K classroom, there was two children without a top and bottom cover or one piece covering.
In the Preschool classroom, there was three children with no bottom cover.Plan of Correction: Will contact parent to bring top and bottom covers and buy them.
Standard #: 8VAC20-780-530-A Description: Based on record review and staff interview, the licensee did not ensure at least one staff in each classroom or area children are present.
Evidence:
Staff #4 was observed in the 2's classroom alone. Staff #4 is not CPR and First aid certified.Plan of Correction: Class is schedule for May 11, 2024.
Standard #: 8VAC20-780-550-F Description: Based on record review and staff interview, the licensee did not ensure a lockdown procedures was practiced at least annually.
Evidence:
A lockdwon drill was not condcuted in 2024.Plan of Correction: Lockdown drills will be done in 2024.
