Comments:
A renewal inspection was initiated on September 12, 2022 and concluded on September 12, 2022. There were seven children present, ranging in ages from Infant to three years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-800-50-A
Description: Based on documentation review, the family day home did not keep and accurate written record of children in attendance each day.

Evidence:
1) On the date of inspection, the inspector observed seven children in attendance, however, the attendance record only had five children documented in attendance for the day.

Description: Based on documentation review, the provider did not ensure that additional immunizations were obtained once every six months for children under the age of two years.

Evidence:
The records for child #3 (birthdate: 6/11/20) and child #5 (birthdate 8/27/21) did not contain updated immunizations every six months.

Standard #: 8VAC20-800-210-A
Description: Based on review of documentation and interview, the provider did not ensure that in addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. The records for staff #1 and staff #2, did not have documentation of annual training hours.
2. The provider stated that staff #1 and staff #2 did not have the required annual training hours.

Description: Based on observation of the family day home, one area of the home was not maintained in a safe condition.

Evidence:
The bottom of the dishwasher is missing the cover exposing wires and other parts to the motor of the dishwasher.
