Comments:
An unannounced monitoring inspection was conducted with center staff on May 31, 2019 to determine compliance with licensing standards. There were four toddlers and nine preschool children present during today's inspection with three staff actively supervising. The children were having rest time. Lunch served today: chicken patty, watermelon, crackers, corn, and milk. There are no medications on site today; several staff are certified in Medication Administration Training. Three staff files and five children's files were reviewed and are in compliance. Evacuation drills have been conducted monthly; the last shelter in place drill was documented 3/19/2019. Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not maintain all areas and equipment in a safe and operable condition. Evidence: 1. In the school age rooms upstairs, there are 3 separate window panes that have separated from the window frame that create a sharp hazard. 2. In the younger school age room upstairs, there is a grey plastic bin holding toys that is broken on one side and creates a pinching hazard/sharp hazard. 3. In the older school age room upstairs, there is a hole in the ceiling where drywall is missing measuring 9 inches in diameter.

Plan of Correction: Repairs will be made as needed.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurements taken, the facility failed to have adequate resilient surfacing under and around equipment on the playground. Evidence: there is bare dirt at the base of two slides, bare dirt at the steps leading to the slides; platform, and bare dirt under and around two Little Tikes slides.

Plan of Correction: The children will not use the equipment that does not have adequate resilient surfacing until mulch can be delivered for the playground.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to have a diaper disposal storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: the diaper pail in the infant room did not open the lid when the foot pedal was pushed. The caregivers's hand touched the exterior of the lid to lift it.

Plan of Correction: The diaper pail will be fixed or replaced.
