Comments:
An unannounced complaint inspection was conducted on 1/19/2024 with the local department of social services to investigate an allegation received by the department related to supervision of children. Additional interviews were conducted on 1/24/2024 and 2/7/2024. Based on evidence gathered through interviews, documentation and a video recording, the complaint was determined to be valid. A violation notice was issued to the facility. An exit interview was conducted on 3/15/2024. Complaint related: Yes
Description: Based on multiple interviews, documentation, and a review of a video recording, it was determined that the facility failed to ensure the children's care, protection, and guidance. Evidence: 1) Between approximately 4:30-5:00pm on 12/21/2023, in the school age classroom, staff #1 was observed sitting at a desk scrolling on their phone while child #1 (age 8) and child #2 (age 10) were inappropriately touching each other. 2) The children were observed on the rug laying close to each other, and moving, while under their coats and a blanket for approximately 20-25 minutes. 3) When staff #1 finally intervened, both children were observed to have their pants down. 4) Staff stated that they intervened as soon as they realized there was inappropriate behavior.

Description: Based on review of a video recording, and observation of the classroom, it was determined that the facility failed to ensure that children under ten years of age are always within actual sight and sound supervision of staff. Evidence: 1) Between approximately 4:00-5:00pm on 12/21/2023, staff #1 was observed walking in and out of the school age classroom three different times, leaving children out of sight and sound supervision for approximately 15-30 seconds each time. 2) Staff #1 was observed walking around the classroom cleaning, and in the book shelf/cubby area in the back corner of the room two different times, leaving children on the carpet without sight supervision for approximately one to two minutes each time. 3) When staff #1 discovered the actions of child #1 and child #2 on the carpet, they took the children to the front office leaving the other 14 children alone in the classroom and out of sight and sound supervision of staff for approximately one minute. The youngest child present was five years old.

Description: Based on interviews and a review of a video recording, it was determined that staff did not use behavioral guidance intended to redirect children to appropriate behavior. Evidence: In the school age classroom, on 12/21/2023, staff #1 did not intervene to stop two children who were engaged in inappropriate behavior for approximately 20-25 minutes.
