An unannounced monitoring inspection was conducted on 10/25/2023. At the time of entrance, 2 children in care with the provider. The record sample size consisted of 5 children's records and the provider's record. The children in care were observed engaging with the provider, participating in free play and napping. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Standard #: 8VAC20-790-170-C Description: Based on record review and interview it was determined the provide did not ensure they submitted a request for a repeat background check in accordance with ? 22.1-289.040 of the Code of Virginia every five years.
Evidence:
1. The background check available for the provider was dated 5/22/18.
2. Provider confirmed they had not submitted a request for an updated background check.Plan of Correction: Provider will schedule an appointment to have an updated background check completed.
