Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before attendance or within one month after attendance.
Evidence:
1. There was no documented physical exam on file for child 2 who has an enrollment date of 09/09/2019.Plan of Correction: The parent of child 2 has been asked to provide a physical. This child is not in care today and has only attended occasionally since enrollment.
The parent will be contacted today in order to request the current physical. The child will not be allowed to attend until the physical is provided.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.
Evidence:
1. The enrollment record for child 1 lacked a work phone number for each parent's place of employment.
2. The enrollment record for child 2 lacked a second emergency contact.Plan of Correction: The work phone numbers for the parent's of child 1 were obtained during the inspection.
The parent of child 2 will be asked to provide a second emergency contact at pick-up this afternoon.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
Evidence:
1. Three electrical outlets in the computer room lacked a protective cover.
a. Children were observed in this classroom during the inspection.Plan of Correction: Protective covers were placed in all of the outlets during the inspection.
Standard #: 22VAC40-185-350-E-1 Description: Based on observation and interview, the center failed to ensure that the following staff-to-child ratios are maintained: For children from birth to the age of 16 months: one staff member for every four children.
Evidence:
1. One staff was observed with a group of 7 infant and toddlers at 1:10 pm.
a. Three of the children in the group ranged in age from 12 months - 15 months.
2. The staff present with this group of children stated that she had been alone with the group for approximately 20 minutes.Plan of Correction: Staff will be immediately reminded that the ratio for this class can not double at nap if infants are present.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the center failed to ensure that the findings of the most recent inspection of the facility were posted on the premises.
Evidence:
1. The findings from the most recent inspection conducted on 00/00/2019 were not posted.
2. The findings from the inspection conducted on 12/21/2018 were posted.Plan of Correction: The findings from the inspection conducted today will be posted.
