Comments:
A self reported monitoring inspection was initiated on 11/1/2023 and concluded on 11/28/2023. There were twelve children present with three staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming. A total of one child record and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A
Description: Based on record reviews the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence:
Staff # 2 was working on 10/27/23 & 11/1/23, the staff record did not have a TB screening on file.

Description: (REPEAT VIOLATION) Based on record reviews the center failed to ensure that program leaders meet the required qualifications.
Staff #1 &2 were working in a program leader capacity on 10/27/23. Both staff do not have the required qualifications to be a program leader.

Standard #: 8VAC20-780-340-A
Description: (REPEAT VIOLATION) Based on interviews the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
Center staff were not aware that an eighteen (18) month old child was left outside the center and was missing for an undetermined amount of time on 10/27/23.
Three staff took 12 children to the playground across the parking lot from the center. Each staff was pushing a stroller with a total of four infants. Eight children walked between the three staff.
The three staff and 11 children went inside the center and the eighteen-month-old child was left outside. While outside of the center, the child walked unsupervised approximately 200 feet away from the center, down the sidewalk. The unattended child was witnessed, by an individual sitting in his car, at the rear of a car in the parking lot. This individual saw a passing car miss the child by approximately six feet. The individual who initially saw the unattended child retrieved the child and carried him back to the center. This individual rang the doorbell, and a staff member took the child into the center. The child was missing for an undetermined amount of time.

Description: (REPEAT VIOLATION) Based on interviews the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
On October 27, 2023, the staff failed to maintain sight and sound supervision of an eighteen (18) month old child.
Three staff took 12 children outside to the playground. The three staff then brought 11 children back to the center. The eighteen month (18) old child did not go inside the center. The child walked approximately 200 feet from the entrance to the center and down the sidewalk/parking lot.
The center staff did not know the child was unaccounted for and missing until an individual notified the center. The staff and children had already come inside when the individual rang the doorbell and returned the child to the staff.

Description: Based on observation the center failed to ensure that crib sheets shall be clean and washed daily.
Evidence:
On 11/1/23 a crib that was occupied by a sleeping infant had a sheet that was stained under the infant?s head.
