An unannounced complaint inspection was conducted on 06/17/2019 from 11:10 A.M.-1:30 P.M. and on 06/19/2019 from 11:15 A.M. -12:30 P.M. in regards to a concern reported to the Valley Licensing Office on 06/13/2019. Four staff records, sign in/sign out sheets, and staff meeting agenda were reviewed, seven staff members were interviewed, and playgrounds were inspected. The information gathered during the investigation does not support the allegation, so the allegation is determined to be "not valid". There was one violation cited not related to the allegation. Standard #: 22VAC40-185-330-B Complaint related: No Description: Based on observation of the toddler playground, the center failed to ensure that all equipment with moving parts or climbing apparatus had resilient surfacing underneath the equipment, and the fall zones around the large climbing and slide structure had less than six inches of loose-fill mulch in the fall zones. Evidence: 1. There was a moveable climbing structure with a slide that was not placed on resilient surfacing. It was sitting on grass, at the edge of the mulched area of the playground. 2. The resilient surfacing around the large climbing and slide structure had less than three inches of loose-fill mulch. The mulch was hard and had compacted. 3. Staff verified that the moveable climbing structure had been on the playground for awhile and that children had been using the equipment. Plan of Correction: Administration removed the moveable climbing structure from the playground. Administration is going to look at the items on the playground and scale back, and add mulch to the areas needed. Administration is also going to research some ideas for a barrier to place around the mulch to help contain and keep it at the required depth longer.
