Comments:
An unannounced complaint inspection was conducted on 12/16/2019 from 2:50 P.M.-4:10 P.M. in regards to a concern reported to the Valley Licensing Office on 11/22/2019. Ten staff members were interviewed, and classroom observations conducted. The information gathered during the investigation does not support the allegation, so the allegation is determined to be "not valid". There was one repeat violation cited not related to the allegation. The exemptions to compliance are noted on the violation notice.

Description: Based on classroom observation and discussion with staff on December 16, 2019, program staff failed to ensure all children under 10 years of age were within actual sight supervision.

Evidence:
1. On 12/16/2019, the licensing inspector (L.I.) observed six children, (age 2), out of sight supervision of Staff #1 in Classroom #1. Staff #1 was changing one child on the changing table, which was located in the adjacent room to Classroom #1. Where the changing table is located you can't see around the room divider and the front part of the classroom. Six children were observed standing at the doorway near the gate of classroom #1. Staff #1 did not have sight of the six children during the diaper change.
2. On 12/16/2019, the Licensing Inspector (L.I.) observed seven children, ages 0-9 months, out of sight of Staff #2 and Staff #3. Seven children were observed in Classroom #2 in bouncers, having tummy time, and playing. Staff #2 was changing a diaper of a child on the changing table that is positioned in the adjoining room. Staff #2 could not see the children from where she was standing changing a diaper. Staff #3 walked around the diaper table to the sink area to prepare a bottle. Staff # 3 could not see the children from the area where she was preparing a bottle.

Plan of Correction: We re-arranged the classroom to ensure that during all times that sight and sound supervision will be maintained. We conducted a meeting with all staff to remind them even with a gate in the class we must maintain sight and sound supervision at all times.
