Comments:
An unannounced Monitoring Inspection was conducted today from 6:20am to 7:15am. There were 9 school-agers directly supervised by 2 staff. The physical plant, 2 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed socializing, playing with blocks, playing Uno, and arriving to care. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on observation, the facility did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: Observed that there was hydrogen peroxide accessible to children in an unlocked cabinet.

Plan of Correction: The hydrogen peroxide was moved to a locked cabinet.

Description: Based on observation, the facility did not have a working battery operated radio.
Evidence: Observed that the facility did not have enough batteries for the battery operated ratio.

Plan of Correction: Will obtain batteries for the radio.
