The Licensing Inspector for Garden of Prayer #7 Great Beginnings Day Care conducted an unannounced RECDC Monitoring Inspection on 03/09/2020. Licensing Inspector arrived & started inspection at 9 am. There a total of 18 children ages 14 months? 5 years old present under the care of five (5) staff members. The Licensing Inspector reviewed information for five (5) enrolled children & information for five (5) staff members. Center does not administer medication and provides transportation for school-age children. Violations found during today?s inspection are identified on the violation notice. LI Kimbrough conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director on March 09, 2020. Standard #: 22VAC40-191-40-C-2 Description: Based on review of staff records the facility failed to maintain compliance with this standard as required related to Sworn Disclosure Statements.
Evidence:
Staff #4, hire date 3/3/20, did not have a Sworn Disclosure Statement as required. This staff was observed taking the children to the restroom during this inspection.Plan of Correction: The staff member will fill out the statement today. In the future, upon hire, staff will complete the form.
Standard #: 63.2(17)-1716-A Description: Based on review of the facility postings the facility failed to maintain compliance with this standard as required.
Evidence:
The Exempt Status letter generated from the DSS was not posted as required.Plan of Correction: A copy of the letter from the DSS will be posted as required and updated annually for complaince.
