An unannounced monitoring inspection, resulting from a self-reported incident received on 04/19/2024, was conducted on 04/29/2024. The inspector reviewed video footage, 1 children?s record, 1 staff record and interviewed staff. The inspector arrived at approximately 11:50 am and departed at approximately 1:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-410-1 Description: Based on video footage, the center failed to ensure that actions towards children that are forbidden by the standards (physical punishment, striking a child, roughly handling, or shaking a child) were not used at the center.
Staff 1 was observed roughly handling Child 1. The video footage shows Staff 1 repeatedly picking up Child 1 off of the ground by one arm.Plan of Correction: -Staff is no longer employed.
-All staff have received additional training on behavior guidance.
