Comments:
A Subsidy Health and Safety Inspection was conducted on 10/15/2019 from 9:45 A.M.-2:00 P.M. to review supplemental health and safety requirements. At the time of the inspection 44 children were in the care with eleven staff and two administrators present. Five children records, seven staff records, one board members background checks, two medications and required documentation, first aid kit and emergency supplies, evacuation drills, parent handbook, six injury reports, injury prevention plan, health inspection, fire inspection, outdoor play area, and required posted documents were reviewed. Children were observed free play, art activities which included coloring and gluing, writing activities, sensory activity, diapering, infant feeding, lunchtime, transition to playground, behavioral guidance, handwashing, and naptime. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during the supplemental inspection. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-665-540-A
Description: Based on review of staff records, the center failed to obtain a tuberculosis screening prior to staff coming into contact with children or within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #4's tuberculosis screening was dated 06/19/2019. Date of hire was 09/30/2019.
2. Staff #5's tuberculosis screening was dated 07/19/2019. Date of hire was 09/13/2019.
3. Staff #7's tuberculosis screening was dated 08/28/2018. Date of hire was 05/20/2019.
3. Staff verified the dates of employment and date of tuberculosis screenings.

Plan of Correction: Administration was aware that the tuberculosis screening needed to be completed before coming in contact with children, but was confused with the thirty day prior to employment as required by subsidy and within date of 12 months as required by Child Day Center requirements. In the future administration will make sure all tuberculosis screening are obtained within date of 30 days prior to employment.
