63.2(17) License & Registration Procedures

Comments:
Two licensing inspectors conducted an unannounced renewal inspection Wednesday, December 11, 2019 from approximately 11:40 AM until 3:00 PM. The children and staff were engaged in a variety of activities while the licensing inspectors were on the premises. Children were observed playing outside, using activity mats, working puzzles, playing in dramatic play center, listening to a chapter book and looking at books. Children were also observed having their diapers changed, taking bathroom breaks, washing their hands, and eating lunch. Lunch consisted of cheese pizza, green beans, pears and milk. Later, children were observed resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play areas were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, injury prevention plan, infant feeding schedules, daily attendance, infant daily sheets, practiced emergency drill logs, liability insurance policy, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected and found complete. Medication and medication authorizations were reviewed. Transportation is not provided at this time. During the inspection, ten children's records and ten staff records were reviewed.

Violations cited during the licensing period were checked for corrections. There was one repeat violation cited during today's inspection. Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of medication and authorizations, the center did not ensure nonprescription medications administered were consistent with the manufacturer's instructions for age, duration and dosage.

Evidence:

1. Medication for child #11 (age 8 months) stated for children under two years ask a doctor for dosage.
2. The medication authorization for for child #11 did not contain dosage recommendation or authorization from a physician.
3. Documentation indicated the non-prescription medication for child #11 had been administered on 12/02/2019.

Plan of Correction: Per the administration: We will contact the parent and ask that they provide a physicians not for dosage of the medication. We will also send out an email for all parents to explain the policy for future reference.

Standard #: 22VAC40-185-570-A
Description: Based on observation and interview, the center did not ensure children under 12 months age placed in a feeding table, used protective belts that were fastened securely.

Evidence:

1. One infant in the light blue class, identified by staff as under 12 months in age, was observed sitting in a feeding table eating lunch. The infant did not have the protective belt fastened.
2. Three infants in the dark blue class, identified by staff as under 12 months in age, were observed sitting in a feeding table eating lunch. The infants did not have the protective belt fastened.

Plan of Correction: Per administration: We will discuss one on one with the staff in those two classrooms our procedures to ensure children as fastened while in feeding tables. We will also review our procedures will all staff as well.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on the review of the ten staff records, the center did not request a search of out-of-state founded complaints of child abuse and neglect within the first 30 days of beginning employment for one staff member who had lived outside of Virginia within the last five years.

Evidence:

1. The records for Staff #4 (hire date 11/05/2019) indicated the staff member had lived outside of Virginia within the last five years.
2. The records did not contain documentation of a request for founded complaints of child abuse from the state the staff members indicated they had lived in within the last five years. 3. Administration stated an out-of-state child abuse search for Staff #4 had not been requested as of 12/11/2019.

Plan of Correction: Per the administration: We will request the out of state background check for the staff member right away and send documentation to the licensing office that the request has been made.
