Comments:

An unannounced monitoring inspection was conducted on July 20, 2022 from approximately 10:32 am to 1:35 pm. There were 160 children in attendance and a total of 45 staff present. 10 staff records and 10 children?s records were reviewed. All areas of the center were observed including classrooms, hallways, kitchen, and bathrooms. The children were observed playing with toys and puzzles, reading, participating in circle time, touring a bus for ?community helper week?, playing outside, washing hands, and eating lunch while the licensing inspector was on the premises. Lunch consisted of macaroni and cheese, broccoli, chicken tenders, pears, and milk. The most recent Health Inspection was on 03/04/22. Fire Drills are conducted monthly with the most recent on 07/06/22. The First Aid Kit had all components. The license, current inspection, and evacuation plan were posted and emergency numbers were posted by each phone. Violations were cited as a result of this inspection. Standard #:	8VAC20-780-60-C
Description:	Based on a review of records, the center did not document viewing proof of the child?s identity and age. Evidence: Two of ten enrolled children?s records viewed were missing documentation of reviewed proof of the child?s identity.
Plan of Correction:	The center corrected this the day of inspection and it is now in the child's file.
Standard #:	8VAC20-780-90--A
Description:	Based on a review of records, the center did not obtain a written agreement between the parent and the center. Evidence: 1 of 10 enrolled children?s records did not contain the written parent agreement.
Plan of Correction:	The center contacted this parent the same day as the inspection and the file was updated.
Standard #:	8VAC20-780-240-A
Description:	Based on a review of records, the center did not ensure that the Virginia Department of Education (VDOE) sponsored orientation course was completed within 90 calendar days of employment. Evidence: 2 out of 10 staff records reviewed, staff #7 and staff #8 did not contain documentation of completion of the VDOE Orientation training course. The date of employment for Staff#7 was documented as 3/7/22. The date of employment for Staff #8 was documented as 2/21/22. All staff hired after 10/13/21 must complete the VDOE orientation course within 90 calendar days of employment.
Plan of Correction:	Moving forward this training has been added to the Orientation process for staff. This was not previously a part of the process as we were not a subsidy provider.
Standard #:	8VAC20-780-260-A
Description:	Based on a review of documentation, and interview, the center did not provide to the Licensing Inspector an annual fire inspection report. Evidence: The requested fire inspection report was not sent to the Licensing Inspector within the requested time frame.
Plan of Correction:	The center has contacted the Fire Marshall and they are behind schedule and unable to send a Fire Marshall at this time. In our understanding this is a State wide issue. In the interim the center will continue a partnership with VCU Fire and Safety to ensure all equipment is functioning appropriately, along with monthly fire drills.
Standard #:	8VAC20-780-270-A
Description:	Based on observation, the center did not maintain equipment of the center in a clean, safe, and operable condition. Evidence: 13 out of 18 classrooms had cabinets that were broken and/or locks that were inoperable.
Plan of Correction:	The center submitted work orders the same day as inspection and the cabinet locks have been fixed.
Standard #:	8VAC20-780-550-E
Description:	Based on a review of documentation, the center did not ensure to conduct two Shelter In Place drills per year. Evidence: The drill log listed fire and lock-down drills but it did not contain any shelter in place drills.
Plan of Correction:	Center administration will ensure a second drill occurs by end of September 2022.
