63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 27, 2021 and concluded on May 5, 2021. The director was contacted by telephone to initiate the inspection. There were thirty-three children in the care of five staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word `corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on record review, program did not ensure that a sworn statement was completed prior to first day of employment.

Evidence:
1. The date of employment for Staff #1 was 3/1/20.
2. Sworn statement in file was dated 5/19/20.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, program did not ensure that results from a fingerprint based background check was completed prior to first date of employment.

Evidence:
1. The date of employment for Staff #1 was 3/1/20.
2. No results were in file.
3. Administrator confirmed lack of results in file, stating that appointment for 3/17/20 was cancelled due to pandemic. Staff #1's ID expired July 2020, and was unable to get DMV appointment until 5/3/21.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, program did not ensure that the results were obtained from the child abuse and neglect registry, criminal history record check, and sex offender registry check from any state in which the applicant has resided in the preceding five years.

Evidence:
1. The date of hire for Staff #3 was 2/16/20.
2. There were no background checks in file from previous state of residency.
