This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 4/20/21 and concluded on 4/21/21. The director was contacted by telephone to initiate the inspection. There were 86 children present and 6 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A Description: Based on a review of four children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information.
Evidence
1. The record for child #3 did not contain the registration form with all of the required information about the child and his parents.
2. Staff #5 (Program Director) confirmed that the records for child #3 did not contain all of the required information.Plan of Correction: The facility responded: We have contacted the parents and asked to them provide all of the required information.
