Comments:
An unannounced monitoring inspection was conducted today August 13, 2019 to determine the facility's compliance with licensing standards. The inspection was initiated at 2:00pm and concluded at 5:00pm. Upon the inspectors arrival children were napping. Eight children were in care with two staff and the program director. After nap children had bathroom breaks and washed their hands before choosing activities that included matching picture cards to letters, playing in the home living area, making fish in an art activity and listening to stories. The summer camp theme this week is fish. Later children had snack that consisted of pretzels and fruit with water. Children provide their own lunch which were labeled with names and today's date and the center provides snacks. Children were also observed during music and movement marching around the room as different animals and playing freeze dance. Staff and children interactions were positive. The main classroom, rest rooms, indoor play room, hallway and outdoor playground were inspected. The classrooms are equipped with age and stage appropriate materials for literacy, art, fine and gross motor skills and creative play. The facility had the required information posted to include the License, last inspection notice, emergency numbers, daily schedule, evacuation maps and various parent information. The first aide kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Six staff records were reviewed during today's inspection along with the records of five children. Per the Director, medications are currently not being administered and are not on site. Staff have current CPR and First aid training. Today's census: (3-5 years old): 8 children with 2 staff. The last emergency evacuation drill was conducted on 07/15/2019. The two required shelter-in-place drills for the 2018-2019 year were documented on 09/17/2018 and 01/15/2019, additional drills were documented on 06/15/2019 and 07/15/2019. Standard #: 22VAC40-185-240-D-5
Description: Based on interview and review of records, the facility did not ensure there was always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: 1. There was no documentation of daily health observation training in the six staff records reviewed. 2. The administrator reported there we no staff with daily health observation training currently employed.

Plan of Correction: Per the administrator: A class has been scheduled to obtain daily health observation training.

Description: Based on review six staff records, the center did not obtain a central registry finding for one staff member within 30 days of employment or volunteer service. Evidence: The record of staff #3, hired 09/25/2017, had a central registry finding dated 11/17/2017 exceeding the 30 days from hire allowed to obtain.

Plan of Correction: Per the administrator: Background checks will requested within seven days of hire and followed up on if not returned within 30 days.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on the review of the six staff records, the center did not request a search of out-of-state founded complaints of child abuse or neglect within 30 days of beginning employment for two staff members who have lived outside of Virginia within the last five years. Evidence: Documentation for Staff #3 (hired 09/25/2017) and Staff #5 (hired 09/28/2018) indicated the staff members had lived outside of Virginia within the last five years. The records did not contain documentation of a request for founded complaints of child abuse from the states the staff members indicated they had lived in within the last five years. Staff #3 was observed working at the center during the inspection. Administration stated an out-of-state child abuse search for Staff #3 and Staff #5 had not been requested.

Plan of Correction: Per the administrator: I will request the two out of state background check today.
