Standard #: 8VAC20-780-60-A
Description: Based on a review of 3 children's records, the facility did obtain documentation of child updates and confirmation of up-to-date information in the child's record as required by 8VAC20-780-420 E 3.

Evidence: The file of child #1 was last signed/dated as updated on 9/12/2022.

Plan of Correction: The classroom teacher has updated to include the correct student file information to reflect the 2023-2024 school year.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation of classrooms facility failed to ensure all electrical outlets have protective covers.

Evidence: The Owls and Tadpoles classrooms had electrical outlets without covers.

Plan of Correction: I have reminded all staff to check their electrical outlets and extension cord outlets as sometimes outlet covers are removed by the building cleaners. I provided them with extra outlet covers.

Standard #: 8VAC20-780-510-B
Description: Based on a review of medications the facility did not have a written authorization from the parent.

Evidence: Child #2 has an Epi-pen on site without an authorization signed by the parent. There was also an authorization on file in the Tadpoles classroom without a child's name on the form.

Plan of Correction: Child #2 information was in our confidential database Child Plus. It has now been printed and placed in the child's file and medication box.
The parent has written their child's name on the Chronic Health and Conditions Care Plan.

Standard #: 8VAC20-780-550-E
Description: Based on review of emergency drills, the facility failed to practice the shelter-in-place procedures a minimum of twice with the past year.

Evidence: The posted emergency drill log did not contain a record of a shelter-in-place drill since 9/1/2023.

Plan of Correction: The licensing board has both the updated record and the previous one that contains our lockdown and evacuation drills.
