Comments:
An unannounced monitoring inspection was conducted on September 19, 2019 from approximately 2:15 PM to 4:30 PM to determine the center's compliance with licensing standards. The child to staff ratios were deemed in compliance with licensing standards. There were 24 school-age children in care at the time of the inspection. There were 4 staff members working directly with the children. The licensing inspector observed children participating in a variety of activities. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. A snack is provided by the facility. The facility had the following posted: license, emergency numbers, evacuation maps/procedures, daily schedule, menu, and various parent information. There were no medications reported at the center. The first aid kit and non-medical emergency supplies were inspected. Emergency response drill records were reviewed. 5 children's records were reviewed. Staff records will be reviewed at the administrative office at a later date. If you have any questions in reference to this inspection, please contact the licensing inspector at 804-662-9092.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records, three records did not contain the required documentation. Evidence: 1. The records for Child #3 (start date 8/26/19) and Child #4 (start date 8/26/19) did not contain documentation of a second emergency contact. 2. The record for Child #5 (start date 8/15/19) did not contain documentation of immunizations or a physical.

Plan of Correction: Administration will request the required documentation from parents.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview, the center did not have a working battery-operated radio on site. Evidence: The licensing inspector observed the center's radio did not contain batteries. Staff interviewed could not locate the radio batteries during the inspection.

Plan of Correction: Administration will obtain batteries for the emergency radio.
