This inspection was conducted by licensing staff using an alternate remote protocol, including
telephone contacts, documents review, interviews (if applicable) and a virtual tour of the
program (if applicable).
A monitoring inspection was initiated on Thursday, March 24, 2022 and concluded on Tuesday, March 29, 2022. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 126 children present with 22 staff supervising and 2 administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on review of children's record, the center did not ensure to obtain documentation of a physical examination within 30 days after the first day of attendance for all children.
Evidence: In Child#4's record (start date 11/15/21), there was no physical examination documentation available for review during the inspection.Plan of Correction: There was a physical examination dated 10/14/21. When scanned in digitally it was not sent as it was on the back of the form. Technological error.
