Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 6/2/2020 and concluded on 6/2/2020. The administrator was contacted by telephone to initiate the inspection. There were 16 children present and 3 staff. The inspector emailed the administrator a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of documentation requested by the inspector and interview, the licensee did not ensure that each enrolled child's record contained all of the required documentation to include the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Evidence:
Two children's records were reviewed. The documentation provided for child #2 contained only one of the two required emergency contacts' name address and phone number.

Plan of Correction: The administrator will request that child #2's parent provide the required information for a second emergency contact and will document that information in child #2's record within 2 weeks.

Standard #: 22VAC40-185-260-A
Description: Based on a review of records and interview, the facility did not ensure that an annual fire inspection report from the appropriate fire official having jurisdiction is provided.
Evidence:
At the previous monitoring inspection of the center on 12/30/19 the most recent fire inspection available for the center was dated 8/27/18. At that time, the administrator stated that a more recent inspection was not available and that she was not aware that she needed to request the inspection annually.
At the current inspection, the administrator was unable to locate an updated fire inspection.

Plan of Correction: The administrator will contact the fire Marshall's office to request an updated fire inspection within 2 weeks.
