Technical Assistance:
Please ensure that a total of 16 hours of annual training are documented prior to the end of the first year following the Vendor agreement. You may include the 2 Virginia Preservice Trainings and up to 2 hours for CPR and First Aid training if the took place int he first 12 months as a Vendor.

Comments:
An announced Subsidy Health and Safety Monitoring inspection was conducted on 2/18/19 from 9:30 a.m. until 11:15 a.m. Inspections are normally unannounced, however, there are no children currently enrolled for care in the home, so the inspector arranged a time that the vendor would be home and available for inspection. The records sample size consisted of 3 previously enrolled children?s records and 2 vendor / caregiver / household member records. The home was inspected - to include the main daycare room, the kitchen where children eat meals and snacks and the restroom used by children in care. When the children play outdoors, they utilize the side yard with direct supervision and close proximity of the vendor. The vendor states that on occasion, she may administer over the counter medication with written parent authorization and may on occasion transport children to or from the home in a vehicle. Requirements for transportation, to include the use of child safety restraints for children under 8 years old were reviewed today. The vendor reports that there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. There are no children currently enrolled who have diagnosed food allergies. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Standard #: 22VAC40-665-170-A
Description: Based on a review of records and interview, the vendor did not obtain documentation that the program has viewed proof of the child's identity prior to attending the family day home.
Evidence:
The records for three children, previously enrolled in the family day home from 5/15/19 through 7/15/19 were reviewed. None of the three records contained documentation that the vendor had seen proof of identification for these children. The children are her niece and nephews.
When asked, the vendor stated that she requested this information at the time of enrollment but it was not received.
These children are not currently enrolled in her care, but plan to be again in the near future.

Plan of Correction: The vendor stated that she will have this documentation before the children are re-enrolled for care in her home.

Evidence:
The records for three children, previously enrolled in the family day home from 5/15/19 through 7/15/19 were reviewed. None of the three records contained documentation of immunizations.
When asked, the vendor stated that she requested this information at the time of enrollment but it was not received.
These children are not currently enrolled in her care, but plan to be again in the near future.
