Comments:
An unannounced renewal inspection was conducted on December 3, 2019 from approximately 4:30 PM to 6:30 PM to determine the center's compliance with licensing standards. The site director was present for the inspection. There were 17 school-age children in care at the time of the inspection. There were 3 staff members working directly with the children. The licensing inspector observed children participating in a variety of activities. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. Afternoon snacks are provided by the facility. The facility had the following posted: license, emergency numbers, evacuation maps/procedures, daily schedule, and various parent information. Medications and authorizations were reviewed. The first aid kit and non-medical emergency supplies were inspected and found complete. 5 children's records were reviewed. Staff records will be reviewed at the administrative office at a later date. If you have any questions in reference to this inspection, please contact the licensing inspector at 804-662-9092.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff emergency contacts, the center did not ensure each staff person present today had emergency contact information. Evidence: Out of three staff present today, two staff did not have emergency contact information documented.

Plan of Correction: Administration will update the emergency information for staff.

Standard #: 22VAC40-185-510-E
Description: Based on a review of one medication, the center did not ensure that the medication was labeled as required. Evidence: The licensing inspector observed the medication was not labeled with the name of the medication, the dosage amount, or the times to be given.

Plan of Correction: Administration will ensure medications are labeled as required.

Standard #: 22VAC40-185-550-D
Description: Based on a review of emergency drill records and an interview, the center did not implement a monthly evacuation drill. Evidence: The licensing inspector observed the evacuation drill record listed drills being practiced on 8/26/19 and 11/26/19. Staff interviewed had not been employed prior to November 2019 and could not provide information on previous emergency drills and could not locate additional records.

Plan of Correction: Emergency drills will be conducted and documented as required.

Description: Based on a review of four injury report records, four records did not contain the required documentation. Evidence: Two injury records did not contain documentation of how parents were notified, when parents were notified, any future action to prevent recurrence of the injury, and the signature of either a parent or a second staff. Two injury records did not contain documentation of any future action to prevent recurrence of the injury and the signature of either a parent or a second staff.

Plan of Correction: Administration will ensure the injury records contain the required documentation.

Description: Based on observation, the facility did not ensure that all documents related to the terms of the license were posted on the premises of the facility. Evidence: The licensing inspector observed the findings of the inspection dated June 2019 were posted at the facility. The findings for the most recent inspection of the facility, dated 9/6/19 were not posted at the facility. The findings of the most recent inspection of the facility are required to be posted on the premises of the facility.
