Comments:
A monitoring inspection was conducted on 5/8/2023 with the licensing inspector on site from approximately 4:20pm to 5:30pm. The center director and center staff were available throughout the inspection to answer any questions. There were 16 children present with 2 staff supervising. Interviews were conducted with staff and areas of the facility were inspected to include all classrooms, bathrooms and the playground areas. A total of 4 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review and an interview, the center did not ensure four of four children's records were maintained and kept at the center.
1. The site supervisor stated that he did not know how to access the center's children's records through the programs electronic record system.
2. The site supervisor was able to show the licensing inspector how to access and review parent or guardian name and contact phone number for child #1, child #2, child #3 and child #4.
3. The licensing inspector was not able to access the center's electronic children's records.

Plan of Correction: We will retrain and make sure staff can pull and we will work on fixing files.

Standard #: 8VAC20-780-70
Description: Based on record review and an interview, the center did not ensure to maintain all of the required information for one of three staff records.
Evidence:
1. The record of staff #3 (employment date: 1/26/2023) did not contain the following required information at the center: name, address, telephone number of a person to be notified in an emergency and health problems that may interfere with fulfilling the job responsibilities.
2. Staff #3 stated that they did not have the required information at the center.

Plan of Correction: We will work on fixing all staff files on site.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. There were several unlocked cleaners located throughout the classroom, some of which were within the reach of children in care.

Plan of Correction: We will purchase locks and make sure they are locked away.

Standard #: 8VAC20-780-550-D
Description: Based on record review, the center did not ensure to implement monthly practice evacuation drills.
Evidence:
The center did not have documentation showing that they practiced evacuation drills during the following months: November 2022, December 2022, January 2023.

Plan of Correction: We will document these consistently. This is a year round school so we won't have documentation when school isn't in session.
