22.1 EARLY CHILDHOOD CARE AND EDUCATION

Comments:
A renewal inspection was conducted on February 27, 2024. The inspector was on site from approximately 3:00 pm until 4:40 pm. The site director was present during the inspection. There were 20 children present, ranging in ages from six years to eleven years, with two staff supervising. Upon the inspector?s arrival, children were in the gym playing a group game of four corners. Later children transitioned to the cafeteria where they worked on homework, had a snack, and participated in activities of blocks, coloring, math worksheets and reading.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five children?s records were reviewed. Three staff records were reviewed remotely on March 1, 2024.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Repeat
Based on review of five children?s records, the center did not obtain documentation of immunizations for one child before the child attended the center.

Evidence:

1. The record of child #1, enrolled 08/22/2023, contained documentation of immunizations dated 09/26/2023, exceeding the first day of attendance.
2. A member of management confirmed the immunization record for child #1 was obtained after the first day the child attended the center.

Based on review of five children?s records, the center did not obtain documentation of a physical before the child?s attendance or within 30 days after the first day of attendance.

Evidence:

1. The record of child #1, enrolled 08/22/2023, contained documentation of a physical dated 09/26/2023, exceeding 30 days from the first day of attendance.
2. A member of management confirmed the physical for child #1 was obtained more than 30 days after the first day of attendance.

Description: Based on review of three staff records, the center did not ensure two staff completed a minimum of 16 hours of training appropriate to the age of children in care.

Evidence:

1. The record of staff #1, employed 09/01/2020, contained 11 hours of annual training for the period of 09/01/2022 through 08/31/2023 when 16 hours is required.
2. The record of staff #2, employed 07/08/2021, contained 11 hours of annual training for the period of 07/08/2022 through 07/07/2023 when 16 hours is required.

Description: Based on observation, the center did not post in a visible place at each telephone, 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center.

Evidence:

1. The number 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center were not posted.
2. Staff #3 stated emergency numbers were in the licensing notebook. The notebook was kept in the storage closet.

Description: Based on observation, the center did not maintain a menu listing foods to be served for meals and snack during the one-week period.

Evidence:

A post-it note located on the licensing board was dated 02/26/2023 and listed sun chips and fruit for snack. There was no posting for today?s snack or the remaining three days for the week of February 26 through March 1, 2024.
