Comments:
Thank you for your assistance during this announced initial inspection. This on-site visit was conducted on July 31, 2019 from 10:30 a.m. through 12:20 p.m. Two staff records were reviewed five records of YMCA Board officers. The facility was toured and the outdoor play area was observed. Some items observed/reviewed during the inspection included: required posted information, parent pick-up procedures, emergency maps and procedures, first -aid kit, emergency supplies and storing of potential medication. Areas of non-compliance may be found on the violation notice. A recommendation will be made for an initial six-month conditional Child Day Center license with a capacity of 200 children and an age range of four years through twelve years. Description: Based on review of first aid supplies, the center failed to ensure all required items were on site. Evidence: 1. Items in the first aid supply kit were reviewed. 2. One digital thermometer, two triangular bandages and one pair of scissors. 3. The site director confirmed the supplies were not in the first aid kit.

Plan of Correction: A thermometer, two triangular bandages and one pair of scissors will be purchased and placed in the first aid kit immediately.
