Comments:
A monitoring inspection was conducted on 6/8/2023 from approximately 11:45 a.m. ? 6:00 p.m. There were 33 children present ranging in ages from 4 months to 11 years old with 11 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition, and transportation. A total of 6 child records and 5 staff records were reviewed. The information gathered during the inspection determined violations with applicable standards. Description: Based on staff record review, the provider failed to obtain the results of a criminal history
record information search from any state in which the individual has lived in the
preceding five years.

Evidence:
Staff record for S1 documented on the sworn statement having lived out-of-state in the last five years. Record for S4 employed 03/13/2023 did not contain a criminal history record information check for that state.

Description: Based on child record review, the provider failed to ensure that one of five immunizations records for children were signed by a physician, designee, or official of a local health department.
