Standard #: 8VAC20-800-100-A Description: Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child 1, enrollment date 07/01/2022, lacks documentation of a physical exam.
2. Child 2, enrollment date 07/22/2022, lacks documentation of a physical exam.
3. Child 3, enrollment date 11/01/2021, lacks documentation of a physical exam.Plan of Correction: The provider stated that a physical exam had been obtained for child 3 - this documentation has been misplaced. The parent will be asked to bring in a new copy if the original physical can not be located.
The provider additionally stated that the parent of child 1 and child 2 has been asked to provide a current physical several times - and will be asked again to provide a current physical exam.
Standard #: 8VAC20-800-80-C Description: Based on record review, the provider failed to document in the child's record the method of verification of the child's age and identity.
Evidence:
1. Written verification of age and identity for child 1, child 2 and child 4 was not on file in enrollment records for these children.Plan of Correction: The provider stated the a birth certificate had been reviewed for all of these children however, documentation had not been indicated on the enrollment forms. The provider stated that written verification will be added to each child's enrollment form.
Standard #: 8VAC20-800-90-C Description: Based on record review the provider failed to obtain documentation of additional immunizations of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Documentation of all required immunizations required for children to receive by the age of two years are not on file for child 1 and child 2.Plan of Correction: The provider stated that she has requested an updated immunization record several times for child 1 and child 2. The parent will be asked to provide this information again.
Standard #: 8VAC20-800-240-A Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition.
Evidence:
1. Three screws were observed protruding from the back of the fence panel play board where the play objects have been attached.
a. The screws have a sharp point and could injure a child's finger if touched.Plan of Correction: The provider stated that the screws had been covered however, the coverings have come off. The provider stated she will make try to find a more long lasting method to cover these screws.
