Technical Assistance:
Technical assistance was provided in the following areas with the provider: background checks, TB screening, the use of DSS model forms; all cribs and baby gates must be compliant with CPSC; the requirements of children's records, physical examinations, immunization records, and proof of identity; evacuation drills should be practiced and documented monthly; the storage of food meals provided must meet USDA requirements.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was conducted on 10/14/20. The provider was contacted by telephone to initiate the inspection. There were two children (7 points) present and one caregiver. The entire home and outside play area along with the equipment to be used by the children was inspected. The Inspector reviewed 2 children?s records and 1 caregiver records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation and interview, it was determined that the provider did not ensure that that all of the required emergency supplies are available in the first aid.

Evidence:
1. The first aid kit for the family day home did not include triangle bandages.
2. Caregiver #1 confirmed that the first aid kit for the family day home did not include triangle bandages.

Plan of Correction: The provider responded: I will purchase or make triangle bandages for the first aid kit for the family day home did not include triangle bandages.

Standard #: 22VAC40-111-770
Description: Based on observation and interview, it was determined that the provider did not ensure that a working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.

Evidence:
1. There was not a working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers during the inspection.
2. Caregiver #1 confirmed that there was not a working battery-operated flashlight and a working portable battery-operated weather band radio during the inspection.

Plan of Correction: The provider responded: I will purchase a working battery-operated flashlight and a working portable battery-operated weather band radio .

Standard #: 22VAC40-111-330-A
Description: Based on observation and an interview, it was determined that the provider did not ensure that a landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

Evidence:
1. When caregiver #1 was asked to provide her landline telephone, she show the Licensing Inspector a cordless phone.
2. Caregiver #1 confirmed that she did not have an operable landline telephone is one that does not require electricity to operate.

Plan of Correction: The provider responded: I will purchase a landline telephone that does not require electricity to operate.

Standard #: 22VAC40-111-540-B
Description: Based on observation and interview, it was determined that the provider did not ensure that Infant walkers shall not be used.

Evidence:
1. The Licensing Inspector observed child #1 moving around the living room in an Infant walker.
2. Caregiver #1 confirmed that child #1 was using an Infant walker.

Plan of Correction: The provider responded: I removed child #1 from the Infant walker during the inspection. I will no longer use an infant walker in my family dya home.
