Comments:
This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A renewal inspection was initiated on 9/20/21 and concluded on 9/23/21. The center owner was contacted by telephone to initiate the inspection. The inspector emailed the owner a list of items required to complete the inspection. An on-site tour of the center was also conducted. There were 19 children present with 6 staff. Children were observed engaging eating lunch and resting quietly during nap time. Handwashing and toileting procedures were also observed. A sample of 2 children's records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The record for staff 1 (date of hire: 9/1/21) does not contain the name, address and telephone number of a person to be notified in an emergency.
2. The record for staff 2 (date of hire: 9/1/21) does not contain the name, address and telephone number of a person to be notified in an emergency or the written information to demonstrate that the individual possesses the orientation training required by the job position.
3. The center owner confirmed that the records for staff 1 and staff 2 are lacking all the required information.

Plan of Correction: The center responded with the following: An emergency contact form has been developed and will be filled out by all staff. Staff 2 completed orientation and the orientation form was filled out and signed but was misplaced and could not be located during the inspection. A new orientation form was filled out and signed and placed in staff 2's record.
