Comments:
This inspection was conducted by licensing staff using an hybrid model, including telephone contacts, documents review, interviews, and an on-site inspection.

A renewal inspection was initiated on January 28, 2022 and concluded on February 2, 2022. The director was contacted by telephone and documentation was requested for review. An onsite inspection was conducted on February 2, 2022 from 2:15 p.m.-4:45 p.m. There were 58 children present, ranging in ages from five years to 12 years of age, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous materials were not kept in a locked location.

Evidence:
1. In Classroom #1 there was a bottle of Juniper Surface Cleaner with Clorox Bleach setting on a shelf in the closet. The closet was not locked.
2. In Classroom #2 there were two containers of Clorox Disinfectant Wipes sitting on the table as you enter the classroom. The table was accessible to children.
3. Both the Juniper Surface Cleaner, and Clorox Disinfect Wipes stated to "keep out of reach of children".
4. Staff verified that the cleaning materials were not kept in a locked location.

Plan of Correction: Staff immediately moved the hazardous materials to a locked location. Administration will retrain staff on the requirements for hazardous materials.

Standard #: 8VAC20-780-550-P
Description: Based on review of injury reports, the center failed to ensure that all required information was contained in the report.

Evidence:
1. An injury report dated 11/12/2021 was missing the method of notification, and prevention for reoccurrence.
2. The injury reports dated 12/03/2021 and 12/10/2021 were missing the date and time the parent was notified.
3. Staff verified that the reports did not contain all the required information.

Plan of Correction: Administration will retrain staff on the requirements for completing an injury report.
