An unannounced renewal inspection was conducted today.
There were 12 children present with two staff.
The children were eating breakfast, watching TV and going on a field trip.

Standard #: 8VAC20-780-70 Description: Based on the review of five staff files, the facility failed to check references on each employee.
Evidence:
1. There are no reference checks on file for staff #1-5.Plan of Correction: The director reported that all references will be checked and documented for all current and future employees.
Standard #: 8VAC20-780-270-A Description: Based on a tour of the playground, the facility failed to ensure that all outdoor equipment is kept in safe condition.
Evidence:
1. The red dome climber is not anchored to the ground and presents a tipping hazard.Plan of Correction: The director reported that the dome climber will not be used by children until it can be anchored in the ground to ensure safe use.
Standard #: 8VAC20-780-330-B Description: Based on a tour of the playground the facility failed to ensure that resilient surfacing shall comply with minimum standards...and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence:
1. The depth of the resilient surface material under the swings is less than two inches and four inches around the dome climber.
2. The use zone for the swings is 10-11 feet, of the required 14 feet , in front and back of the swings.Plan of Correction: The program director reported that the equipment will not be used by preschool children in the licensed program. The playground is being moved to another area and will be completely renovated.
