An unannounced monitoring visit and facility tour was conducted on 4/26/24. At the time of inspectors' arrival there were 15 infant to preschool aged children in care with 3 staff members and 2 administrators. Children were observed interacting with staff and nap. A sample of 5 children's records and 5 staff records were reviewed. Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-340-F Description: Based on observation and interview it was determined the center did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.
Evidence:
1. There were 8 preschool aged children on their cots with no staff in the room.
2. Staff #5 was in the kitchen adjacent to the classroom doing dishes.

Standard #: 8VAC20-780-440-L Description: Based on observation and interview it was determined the center did not ensure that pillows and filled blankets were not used by children under two years of age.
Evidence:
1. There was an infant in a crib laying on a boppy type pad with a filled bumper around the edges.

Standard #: 8VAC20-780-450-A Description: Based on observation an interview it was determined the center did not ensure that children other than infants during the designated rest period had linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
