An unannounced complaint inspection was conducted on January 19, 2023 in response to a complaint received by the agency on November 21, 2022 for allegations regarding physical plant, staff supervision, and pets. Interviews were conducted with the staff relating to the allegations, and one pet file was reviewed. The information gathered during the investigation does support the allegation, so the complaint is determined to be valid.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-270-C Complaint related: Yes Description: Based on observation, the program failed to ensure that the temperature be maintained no lower than 68 degrees Fahrenheit.
Evidence:
1. Two of the six thermostats in the facility were set at 65 degrees.
2. The administrator confirmed that their procedure is that thermostats are set at 65 degrees for overnight, and that first staff to arrive increase the thermostats to 68 degrees, but a few were forgotten.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-350-B-1 Complaint related: Yes Description: Based on interviews, the center failed to ensure that the required staff-to-children ratio for children birth up to 16 months (1:4) was met.
Evidence:
1. All staff and administrators interviewed stated there had been issues recently meeting ratios in the infant rooms, with Tuesdays and Thursdays being the most problematic.
2. The administrator stated there had been a meeting with infant staff on January 19, 2023 to discuss staff scheduling in order to address the ratio issues.Plan of Correction: Not available online. Contact Inspector for more information.
