An unannounced complaint inspection was conducted on 12/18/2023 in response to allegations received by the licensing office on 12/11/2023 in the areas of supervision. The inspection concluded with an electronic record review on 12/21/2023. There were 33 children, ages 5 years -11 years, and 6 staff providing direct care and supervision to children present during the inspection. Staff were interviewed and a total of 4 children?s records and 5 staff records were reviewed. The inspector arrived for the inspection at approximately 2:45 pm and departed approximately 3:45 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Based on interviews and observation, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
1. Staff 2 confirms an incident occurred on 12/08/2023 where Child 1 was located by public school staff in restroom A while out of actual sight and sound supervision of program staff.
2. At approximately 3:02 pm on 12/18/2023, the inspector observed Child 3 and Child 4 leave restroom A and return to the cafeteria out of actual sight and sound supervision of program staff. Restroom A is approximately 85 feet from the cafeteria.
3. At approximately 3:03 pm on 12/18/2023, the inspector observed Child 2 leave the cafeteria, enter restroom B, and then return to the cafeteria out of actual sight and sound supervision of program staff. Restroom B is approximately 105 feet from the cafeteria.Plan of Correction: Staff will ensure children are within actual sight and sound of staff.
