Comments:
An announced mandated initial inspection was conducted from 10:30 a.m. until 12:45. No children were in care. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The center had the needed supplies, toys and equipment to begin operation for ages infant through school-age. The inspector discussed with the applicant licensee the need for safety procedures and close supervision of children due to the location of the center being on a main thoroughfare.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Thank you for your cooperation.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the equipment inside and outside the center was not maintained in a safe and operable condition.
Evidence:
1. In the front pre-k classroom, there was one blue and one red faucet in the bathroom accessible to children that still had water in them.
2. In the upstairs school-age classroom, there was a tv mounted to the wall that was plugged in and the cord was accessible to children.
3. The outside wooden steps and platform had the following: 1. The platform to reach the steps was splintered at the bottom board at the corner. 2. There was a nail exposed on the platform and on the fifth step up there was a nail exposed and the step was splintered. 3. The railing had a split on it approximately 2 feet long.

Plan of Correction: 1. The faucets are not being used and I will make sure they are turned off and empty before children are in care.
2. The t.v. will be removed and not used until I can find a way to use the t.v. in a safe manner.
3. All of the issues with the steps and platform will be repaired before children are in care. This entrance will not be used on a regular basis by children.

Standard #: 8VAC20-780-550-C
Description: Based on observation, the center did not have emergency evacuation and shelter in place procedures or maps posted in a location conspicuous to staff and children ono each floor of the building.
Evidence:
1. There were not procedures or maps posted.

Plan of Correction: The procedures and maps are being revised; they will be posted before children are in care.
