Comments:
An unannounced non-mandated inspection, as required by the Conditional License, was conducted on 10/19/2022 for the Y After School at Grandin Court Elementary. There were 25 children, ages 5 -11 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records were reviewed. Staff records were reviewed during the initial inspection. The children were observed during active time on the playground. The inspector arrived for the inspection at 4:00pm and departed the center at 4:48pm.

Discussion on first aid and emergency supplies.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

The inspection findings have been updated as a result of a review completed by the Licensing Administrator on 3/1/2023

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on the review of four children?s records, the center failed to ensure that each child has received the immunizations required prior to the first day of attendance.

Evidence:
1. Child #1 started on 8/23/2022 and on the day of the inspection did not have the required immunization record.
2. Child #3 started on 8/24/2022 and on the day of the inspection did not have the required immunization record.

Plan of Correction: Provider will obtain immunization record from parent.

Standard #: 8VAC20-780-140-A
Description: Based on the review of four children?s records, the center failed to ensure that each child shall have a physical examination record within 30 days after the first day of attendance.

Evidence:
1. Child #1 started on 8/23/2022 and on the day of the inspection did not have the required physical examination record.
2. Child #3 started on 8/24/2022 and on the day of the inspection did not have the required physical examination record.

Plan of Correction: Provider will obtain physical examination record from parent.

Standard #: 8VAC20-780-70
Description: Based on observation, the center failed to ensure the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.

Evidence: Emergency contact information for each staff was not available onsite.

Plan of Correction: Provider will make sure an emergency contact is available onsite for each staff person.

Standard #: 8VAC20-780-550-D
Description: Based on observation and discussion with staff, the center failed to implement a monthly practice evacuation drill.

Evidence: Documentation of monthly evacuation drills was unavailable and on the day of the inspection there have been no emergency evacuation practice drills completed during the first two months of the school year.

Plan of Correction: Provider will ensure practice emergency evacuation drills are completed monthly.
