Comments:
An unannounced monitoring inspection was conducted on 3/18/2024 for the Y After School at Grandin Court Elementary. There were 29 children, ages 5 -11 years and 4 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed. The children were observed during arrival, active time in the gym, handwashing and snack. The inspector arrived for the inspection at 4:06pm and departed the center at 5:03pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-150-B
Description: Based on the review of three children?s records, the center failed to ensure that immunization records shall be signed by the physician.

Evidence: Child #2 start date was 8/18/2023 and on the day of the inspection did not have a signed immunization record.

Plan of Correction: Provider will obtain signed immunization record.

Standard #: 8VAC20-780-70
Description: Based on review of three staff records, the center failed to ensure that all records were complete.

Evidence: Staff #2 start date was 9/26/2023 and on the day of the inspection did not have: name, address and phone number of a person to be notified in an emergency; documentation to demonstrate education; information about any health problems that may interfere with fulfilling the job responsibilities.

Plan of Correction: Provider will update employee records.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: Two spray bottles of cleaning solution were observed in a milk crate to the right of the cafeteria exit door. Three children were observed in the area and it was accessible to children.

Plan of Correction: Provider will ensure cleaning solutions are kept in a locked place using a safe locking method.
