Comments:

A monitoring inspection was conducted on 01/08/2024 from approximately 10:41 a.m. to 6:00 p.m. There were 148 children present ranging from 4 months old to 5 years old and 23 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Ten child (10) records ten (10) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on record review, the provider failed to ensure that all applicants for employment shall submit to fingerprinting and undergo a national criminal background check prior to employment. Evidence: The record for staff S6, employed 06/29/2023 did not have results of a national criminal background check prior to the first day of employment. The results of a national criminal background check are dated 07/06/2023.

Description:	Based on staff record review, the provider failed to obtain the results of a sex offender registry search, out of state criminal history name check, and out-of-state child abuse and neglect search from any state in which the individual has lived in the preceding five years. Evidence: Staff record for S7 documented on the sworn statement having lived out-of-state in the last five years. The record for S7, employed 07/28/2023 did not contain a sex offender searches for two states in which the individual lived in the preceding five years, the results of the out-of-state child abuse and neglect searches, and a criminal history name check for one of the two states the individual lived in the preceding five years.
