Comments:
A Subsidy Health and Safety Inspection was conducted on Wednesday, January 31, 2024. Two inspectors were on site from approximately 10:27 am until 12:18pm. During the inspection, there were a total of nine children in care, totaling 25 points, in the direct care of two caregivers. The children were observed during free-play, dancing, singing, and watching television. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The caregivers were observed having positive interactions with the children in care. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, medication is not administered and transportation is not provided at this time. Two caregivers' records and six children's records were reviewed.

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the `plan of correction? and `date to be corrected' for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-790-190-B-12
Description: Based on a review of six children's records and interview, the vendor did not obtain a written care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record of Child #2, enrolled on 06/01/23, indicated the child has a diagnosed food allergy, but the record did not contain a written care plan. During interview, Caregiver #1 reported a written allergy care plan was not on file for Child #2.

Standard #: 8VAC20-790-250-F
Description: Based on a review of two caregivers' records and interview, the vendor did not ensure one caregiver completed the department's health and safety update course.

Evidence: The record of Caregiver #1 did not contain the department's health and safety update course. During interview, Caregiver #1 confirmed she has not completed the course to date. The department's health and safety update course should be completed annually.

The family day home was approved as a vendor on 01/25/23, therefore, the annual training should have been completed no later than 01/25/24.
