Comments:
A monitoring inspection was conducted on July 7, 2023 with center management. There were 58 children present, ranging in age from 3 months to 5 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of six children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 2 (date of enrollment 6/05/2023) did not have documentation of a physical on file.

Plan of Correction: Physical received on 7/7/23.

Standard #: 8VAC20-780-60-A
Description: Based on review of 6 children's files, the facility failed to have a written care plan for each child
with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child A has diagnosed food allergies, but did not have a completed allergy action plan on file.

Plan of Correction: The parents were notified on 7/7/23 and will have the allergy action plan completed.

Standard #: 8VAC20-780-320-B
Description: Based upon observation, the facility failed to have hand washing sinks that have warm water that does not exceed 120 degrees Fahrenheit. Evidence: The hot water temperature at the hand washing sink in the Duck A classroom reached 132 degrees Fahrenheit.

Plan of Correction: The water heater was lowered and fixed to the appropriate temperature below 122 on Friday, July 6th at 4:00 pm
