Comments:
A mandated renewal inspection was conducted on October 24, 2023 from 12:00 P.M.-12:45 P.M. at administrative offices and from 3:40 P.M.-4:45 P.M. on site. The director/provider was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, four staff records, and five agents background checks were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-40-D-1-a
Description: Based on review of agent background checks, the center failed to ensure that a central registry finding had been completed by end of the 30th day the individual took office.

Evidence:
1. There was not a completed central registry finding on file for Agent #1.
2. Administration verified that there had been evidence that the central registry check was sent off in April 2022 and came back with missing information and was not sent back in to complete the background check.

Plan of Correction: A central registry check for Agent #1 is being obtained and will be submitted by the portal.

Moving forward, the afterschool financial manger will coordinate with the associate executive director to ensure all board members have a central registry completed. A copy of the form will be made, the date it was submitted documented on the form, and make a note to check on the status 30 days after it was submitted to ensure it is returned.
